PURPOSE(S):
This information is being collected and maintained to contact individuals for deployment in the event of a disaster, as well as to ensure their safety while on deployment.
ROUTINE USE(S):
The information on this form may be disclosed as generally permitted under 5 U.S.C. § 552a(b) of the Privacy Act of 1974, as amended. This includes using this information as necessary and authorized by the routine uses published in DHS/ALL – 014 Department of Homeland Security Emergency Personnel Location Records System of Records, 73 Fed. Reg. 61888 (October 17, 2008), OPM/GOVT-001 General Personnel Records System of Records, 77 Fed. Reg. 73,694 (December 11, 2012), and DHS/ALL – 014 Personnel Emergency Contact Information System of Records, 81 Fed.
